Helping You Get Your Life Back

Targeted Case Management is a Medicaid-funded service that uses providers who are experts in their communities to help people will finding stability and to create more independent, fulfilled lives. Qualified recipients can receive the support of a Targeted Case Manager who can work one-on-one with them to secure employment, find stable housing, arrange transportation, work to achieve financial stability, and many other community- and service-related services. The end goal is to help you get your life back – and know you’re not going through difficulties alone.

Our Case Managers work with local community partners like DCBS, court systems, probation and parole services, Youth Service Centers, food pantries, hospitals, churches and more – all to make sure clients’ needs are met.

Individuals who are insured by WellCare and Anthem Medicaid are eligible to receive Case Management services from Revive Wellness.

Clients enrolled in the Revive Wellness Intensive Outpatient Program automatically receive Case Management as part of their care.
